A source says where a claim came from. Agents can prefer concepts with sources over unsourced ones, and humans can audit upstream material.
Verification events

Each event records that a named party checked the content at a date. Multiple events build a review history without rewriting the document.
Trust tiers
Readers derive a tier from the metadata: unverified (no signals),
machine-confirmed (checked by tooling), human-reviewed (a person signed off).
Renderers display the tier; agents can weight retrieval accordingly. A stale
concept - one whose staleAfter date has passed - announces its own decay
instead of poisoning sessions silently.
The habit to build: when you correct an agent, record the correction as a verification event. The next session inherits the lesson.
